For all companies seeking authority to provide certificated telecommunications services in Wyoming pursuant to Wyo .Stat. § 37-15-201. The certificated form is for telecommunications companies regulated by the Commission and will offer local exchange services and/or have local facilities and therefore, are required to obtain a Certificate of Public Convenience and Necessity (CPCN) i.e. Local Exchange Companies (LECs).
For companies seeking to provide telecommunications services in Wyoming pursuant to the Wyo. Stat. §§ 37-15-101 through 37-15-413 and are not regulated pursuant to Wyo. Stat. § 37-15-104. The non-certificated and non-regulated form is for telecommunications companies that are not required to obtain a Certificate of Public Convenience and Necessity pursuant to Wyo. Stat. § 37-15-201 and are only required to register with the Commission for purposes of contributions to assessments levied (Wyo. Stat. § 37-15-501 and Wyo. Stat. §§ 37-2-106 through 37-2-109) and annual reporting (Wyo. Stat. §37-15-407 and Commission Rule, Chapter 4, Section 4(a)) by the Commission and the universal service fund i.e. interexchange (IXC) carriers, wireless, VoIP, radio, payphones, etc.
The informational form is for telecommunications companies registered with the Wyoming Secretary of State and are intending to do business in Wyoming that may or may not be regulated by the Commission and the company is notifying the Commission of its intended entrance into Wyoming. The company informational form is used to set up a Docket Management System number with the Commission for assessment purposes. This form is used for companies not sure of reporting and assessment requirements and allows the Commission to determine what reporting and assessment requirements are necessary, if any.
The Update Form is for telecommunication companies’ already on file or registered with the Commission that need to update or change information. Please complete the sections the Company would like to update with the Commission. If there are no changes to a certain section please check “No Change” in the section. If there are changes to a certain section please check “Replace Previous Contact” in the section, if applicable.
Please note: Changes to anything other than address and contact information should be requested via an application with appropriate supporting documentation included. Changes to update your company’s price schedule URL should be done in writing on company letterhead, which should be in a searchable pdf document with a clickable link.
Is required pursuant to Commission Rule Chapter 3, Section 3 and Commission Rule Chapter 4, Section 4(e) for all regulated telecommunications companies and should be submitted to the Commission each January and July, even if there aren’t any changes to the previous submittal. Additionally, this list should be updated as soon as any contacts change.
Is required pursuant to Commission Rule Chapter 4, Section 4 for all regulated telecommunications companies and should be completed within 24 hours of the initial SIRT notification or as otherwise directed by the Commission. The initial notification shall be reported to the Commission within two hours of onset of a reportable event.
